Position Available: Housing Readiness Case Manager

Company Overview:
Openwork is proud to support the HAY Center in their search for a dedicated Housing Readiness Case Manager. The HAY Center is committed to empowering youth and providing critical support as they transition to independent living.

Position Summary:
As a Housing Readiness Case Manager, you will play a pivotal role in preparing youth ages 17.5 to 24 for successful housing and leasing. This role involves guiding clients through housing assessments, voucher applications, and the leasing process, ensuring they are well-equipped to secure and maintain housing.

Key Responsibilities:

  • Conduct housing assessments to evaluate readiness and provide tailored case management and supportive services.
  • Assist youth in completing housing applications, navigating the voucher process, and attending housing briefings.
  • Manage documentation and submissions to local housing authorities for initial housing processes and recertification's.
  • Support youth during the move-in process at the HAY Center Campus and provide ongoing case management for residents.
  • Participate in wraparound meetings and collaborate with internal programs, focusing on the In-care Program.
  • Maintain accurate case files and complete required data entry.
  • Engage in HAY Center and Housing Continuum Events as needed.


Qualifications:

  • Bachelor’s degree in Social Services, Psychology, Social Work, Criminal Justice, or a closely related field.
  • Knowledge of case management principles, practices, and documentation.
  • Reliable transportation.
  • Ability to pass a background check and drug screening.
  • Proficiency with Microsoft Office.


Preferred Experience:

  • Experience with public housing and housing subsidies.
  • Familiarity with the transitional needs of youth aging out of foster care.


What We Offer:

  • Work Schedule: Monday – Friday; potential weekend work to be discussed during the interview.
  • Compensation: $22.46 per hour with weekly pay.
  • Contract Duration: Through May 2025.
