What You’ll Do:

Provide skilled, compassionate care in the home setting
Follow and contribute to the individualized plan of care
Monitor, document, and report patient conditions using electronic charting
Educate and support clients and families
Participate in ongoing training and collaborate with your care team

What You’ll Need:

Active LPN or RN license in Missouri or Compact State
Availability for one or more of the following: days, evenings, overnights, weekends
Strong communication and clinical decision-making skills
A compassionate, reliable, and professional approach
Ability to work independently and responsibly
