Job Title: QI Nurse- Home Health Care

Job Summary:

We are seeking a qualified and experienced QI Nurse to join our Home Health Care Agency. The QI Nurse will be responsible for ensuring that our agency maintains a high standard of care and compliance with all policies and regulations. This role will involve reviewing patient records, conducting audits, providing feedback to staff, and implementing quality improvement initiatives.

Responsibilities:

• Conduct regular reviews of patient records to ensure accuracy and compliance with regulations
• Conduct audits of staff performance and documentation to identify areas for improvement
• Review oasis to enhance outcomes and process measures
• Identify correct coding and sequencing of home health diagnosis codes
• Provide feedback and training to staff members to ensure high-quality care deliver
• Develop and implement quality improvement initiatives to enhance the level of care provided by the agency
• Collaborate with other members of the clinical team to ensure the highest level of patient care
• Stay up-to-date on the latest regulations and best practices in home health care

Requirements:

• Registered Nurse (RN) license in the state of practice
• Minimum of 2 years of experience in home health care
• Strong understanding of regulatory requirements for home health care agencies
• Experience with developing, implementing, and evaluating CQI programs
• Certified Oasis Clinical Specialist (COS-C) or Home Care Specialist (HCS-O)
