One of our clients, a large physician organization, is looking for a Registered Dietician Nutritionist to join their team.

The position is full-time with a great compensation and benefits package.

Job Description:

Our client is looking for a Registered Dietitian Nutritionist who is passionate about helping patients with chronic conditions who are at risk for disease progression, complications, and cognitive and functional decline.

In collaboration with the physician and interdisciplinary team, the Registered Dietician/Nutritionist will develop and implement an individualized care plan designed to promote the patient’s understanding and management of their condition, optimize quality of life, and improve clinical outcomes.

This role will also develop various nutritional programs (adult focused) for the provider organization. This position is mostly telephonic and telemedicine with quarterly on-site team meetings and rare patient home visits (with advanced notice).

Responsibilities & duties:

  • Conduct comprehensive nutritional assessment, education, and management to patients/clients referred by physicians and/or multi-disciplinary care team.
  • Provide instruction to clients in the use of modified diets.
  • Coordinate, implement, and evaluate nutritional education using national standards and guidelines.
  • Evaluate patients for needed services (HEDIS metrics) and assist in closing “gaps” for needed health services (preventative and condition-specific).
  • Develop and deliver educational programs and appropriate teaching tools for various ethnic and cultural groups.
  • Provide outpatient and community-based education and counseling to patients and families, individual or group.
  • Prepare nutrition-related presentations and present them to various audiences.
  • Maintain timely and complete documentation, update care plan as appropriate and adhere to billing guidelines of all care management encounters.

Required Qualifications:

  • Bachelor’s degree with course work approved by the American Dietetic Association’s Commission on Accreditation for Dietetics Education.
  • Passed a national examination administered by the Commission on Dietetic Registration.
  • Knowledge of nutrition guidelines in chronic disease management.
  • Knowledge of medical terminology.
  • Knowledge of HEDIS or NCQA quality measures.
  • Healthcare Basic Life Support certification.
  • Valid unrestricted State of MI Drivers License and ability to drive to multiple locations.

Performance Skills:

  • Proficient in Word and Excel, EMRs, and patient registries.
  • Ability to work independently.
  • Ability to work in collaboration within a multi-disciplinary team.
  • Excellent interpersonal and communication skills.
  • Excellent problem-solving, analytical, and decision-making skills.
  • Ability to develop relationships with representatives from other healthcare providers.
  • Knowledge of principles and case management guidelines.
  • Ability to recognize and address the unique culture, language, health literacy, and age-appropriate care of diverse patients and caregivers.
  • Advanced skills in patient interviewing and assessments.
  • Current knowledge of standards of care and clinical guidelines for patients with chronic illness.
  • Ability to work autonomously, maintain work schedule at multiple sites, prioritize, and maintain productivity through proactive case finding and referrals.
  • Working knowledge of insurance and regulatory guidelines for practice.
