About the Role:

As a Full-Time Occupational Therapist in Oakland (Lake Merritt/Piedmont area), you'll partner with the Case Manager to provide skilled services to individuals in their homes. You'll evaluate patients' needs and develop personalized care plans, focusing on areas like home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease management, dementia care, and caregiver training. You'll act as an educator, patient advocate, and valued member of the interdisciplinary care team. This is a hybrid role, requiring travel to patient homes.

Conduct in-home evaluations and develop personalized care plans.
Help patients improve their ability to perform daily activities and regain independence.
Provide education and support to patients and their families.
Collaborate with the interdisciplinary team to ensure coordinated care.
Advocate for patients' needs and connect them with appropriate resources.

What You'll Need to Succeed:

Current unrestricted CA Occupational Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources.
Understanding of individual development and human behavior related to illness/injury, and the influence of culture on healthcare.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Home healthcare experience (highly preferred).
Experience with high-acuity patients (a plus).
Experience working with an interdisciplinary team (highly desired).
Case management fundamentals (highly desired).
Self-directed, empathetic, and a critical thinker.
Outstanding interpersonal and leadership skills.
Organized and detail-oriented.
Compensation:

Starting pay is commensurate with relevant experience above the minimum requirements.
